LGBTQIA2S+ Conference Fee Waiver

  • Please include a reference (name/email) who would be willing to fill out a short form of reference on your behalf.

  • Please upload an essay (750 words) that includes: a) applicant’s motivation for attending the DONA Summit b) how the applicant plans to use the skills learned at the Summit to benefit their community and the short- and long-term goals following the Summit completion c) description of events or circumstances that have shaped your desire to attend the Summit and will help us know more about you d) what you believe will be your potential contribution to your community after attending the Summit. List at least three (3) of your long-term goals as a doula in your community. Max file 50MB
